Sheppard Software has several interactive games for all concepts.

IXL has math and reading activities for K-12 students.  You have to pay to use the website, but they do give you a certain number of free activities without an account.  You can sign up individually as a class or as a whole school.  My students really enjoyed using IXL and it breaks everything down by standards for you.

Prodigy- is a RPG game based math program.  With Prodigy, learning is a whole classroom experience. Unlike other websites (e.g., coolmathgames, coolmath, or multiplication.com), students can play and learn through an ever-expanding world with their friends. Their progress is automatically saved, so students will run home at night to continue playing with their classmates!

Fraction Calculator - This page hosts a fraction calculator that can perform addition, subtraction multiplication or division of two fractions. The values for the calculation can be simple or mixed fractions, or consist of only wholes.

Percentage Calculator - This online percentage calculator shows how a percentage is represented as a fraction of a number. To find a percentage of a number out of a total, simply enter the values in left side of the calculator with the partial number as the numerator (the value on the top) and the total as the value on the bottom (the denominator). The calculator will calculate percentage value numerically, and show you the same value as represented visually.

Cyberbee has some great math activities for all different math concepts.  There are games that you can play as a class or individually.  They give you step by step instructions to complete each activity so the students learn about the concept and then practice it.
